I have some papers to fill but there are some problems.

In my country most doctors don't have good english ability so it can be a little problem with filling this form. So i was thinking about it for a while and i have some questions.

Lungs conditions <- Is "Lungs are healthy" + health exam date ok? Do I really need this Xrays?

座高 <- can anoyone describe how to measure this funny atribute? big_smile

Adress <- clinic adress? doctor adress?

also, I have 国籍, should i write here ポーランド OR ポーランド人

I did not get an x-ray; the consulate told me it was OK just to have a regular exam.

座高 is bizarre -- I don't remember that on my form.  I guess you can use a measuring tape or something?

Address should be clinic address.

For 国籍 I think either is OK?

座高 usually uses one of these: http://www.cs-labo.jp/images/shouhin/NV … 501901.jpg and is measured as such: http://www.fumira.jp/cut/kodomo/img/zakou.gif
So yeah, you could probably use a measuring tape if you wanted to.

For 国籍 I've always written 'USA' on everything, and I've often seen them correct it to 米国, so probably ポーランド is OK

Yeah my wife keeps telling me "イタリア人国籍じゃなくて、イタリア国籍だよ"...I'm such a birdbrain. So, I guess you should write ポーランド...but I don't think they will care.
I got an x-ray exam in order to apply (they told me it was mandatory) and another one just few days after landing in Japan, for that stupid 健康証明書 (without that I couldn't enroll any class = no scholarship). I hope you don't have to do the same...

Yea, my mother said sth like that.

And I went to doctor and said "i'm applying for grant to go to Japan, I just need this form filled and signed by some doc. I have blank form but ... there is also filled by me." He looked at it and wrote a pulse without even checking it xD. In this Xray column i just wrote "Lungs are healthy." tongue. I hope it will make it. He signed it, now i'm filling the rest of this shitty papers.

lol. It's 2 weeks trip so I doubt there will be any more physical exams.
