To make it easier to study failed kanji, it would be nice to have the failed kanji page (study/failed-kanji.php) to have an interface that's more like the "all my stories page" (study/my-stories.php).
Not sure if it's possible, buy maybe have two mouse over areas. Mouse over the text area to display the story, and then mouse over the kanji area to show both the story and kanji.
Another idea would be to display only the failed kanji from the "all my stories" page.

I guess this is a difference in how we view the 'failed' list. My take on 'failed' was that I wouldn't click the 'learned' button unless I could go from the keyword to the kanji (otherwise I haven't learned it, have I?). So I'd look at the keywords on the failed list and if I could remember the kanji I'd mark it 'learned', otherwise I'd go and look at the study page and review/improve the story. So for me any presentation of the failed-list which doesn't hide the story isn't much use, because it already shows you the answers.
Having said that, here's my suggested UI:
My personal preference for handling failed kanji would be to have the study page have a mode where the left-right arrow buttons take you to the next/previous kanji in your failed list. Also, the list of failed kanji on the left of the study page ought to be more than 10 long and ought to always show the kanji you're currently looking at (and the ones around it on the list). Finally, clicking 'learned' should take you to the next kanji on the failed list, not back to the first in the list.
The idea is that you can easily step through the failed list, either marking them as 'learned' or just stepping forward according to whether you learned them or not.
But then I've finished RTK now and hope not to build up a huge failed list again so I'm not too fussed :-)

I love the "all my stories" functionality. I don't find the current failed kanji functionality enhances my memory much. On the other hand, the "all my stories" page is great for memorizing.
Ideally, yes, it would be great to have the failed kanji page show keyword, then story, then kanji. However, adding the "all my stories" listing and hovering functionality only would be a drastic improvement and speed study time.
Basically I work like this: I look at the keyword, deliberately not looking that the story. I then write the kanji. If I can't remember. I look at the story as well. Then I write the kanji or give up. After writing all the kanji listed on that page, I go through and hover over each. I repeat the entire page later if any were missed. If I had a "learned" button, I could start deleting them off the page which would improve speed.
The "all my stories" functionality is awesome but cannot be used to review failed kanji efficiently at this point.
[It would also be nice to be able to hover in the testing phase to see the story as Heisig suggested himself but this is lower priority than the above]
