Crew cut would be ether スポーツ刈り(がり) or 角刈り（かくがり）. スポーツ刈り has slightly longer front, and 角刈り has very square shape as the name suggest.
Actually, like Jarvik7 said, 「軍人（ぐんじん）みたいにしてください」 may best describe it, though I would put 「アメリカの」 in front so I wouldn't end up with Japanese 軍人 style

フケ用 huh? Good to know! I'll keep that in mind for my next trip to a place that has shampoo!
