The most effective method ive found in my (what seems to be a few years of study) is lingq.com with anki (ie: using anki to save sentences and listening to his podcasts throughout the day for listening comp).. and bits of TV shows. LingQ in the morning, SRS at night or in the day with android anki.

This was after doing remembering the hanzi to 1500 chars and adding as i went after that (simply cos i got bored of it)... also watch bits of TV here and there. I think this could be, in a nutshell, a good path to fluency after 18 months or so depending on intensity.
