Does anyone every listening or read something in Japanese and know the words in the sentence but you don't understand what the sentences means? This is been happening more often than not for me. What are some ways of getting over this?

Read more, and try to look up some of the waffle between words: a combination of particles may have a different meaning than you'd expect.
Haha, I know how it feels... I often end up reading on "autopilot" whenever I get slightly distracted, and then realise after half a page that I didn't follow anything at all. It happened a lot when I read for tests (in my own language) back in school as well, so it isn't really a comprehension issue...

The more grammar you study it will be the other way around. You know all the grammar, but the sentence has unknown words which will hinder your understanding of the sentence. However, with context you may be able to understand the word. I find it much better to not understand the sentence due to the vocabulary instead of the unknown grammar.

All the time, yup. What most often happens is I'm just not sure what it means, even if I have one (...or two) decent guesses. I think it's a matter of just continuing to study grammar, including slang or more casual patterns too, as well as keeping reading. Sometimes I've found I didn't understand a sentence, but have later got it after being exposed to the grammar pattern more.

This rarely happens to me. What are you reading?
If I find it's happening, it's usually a good sign that the book is above my level, so I put it down and get a different one. I have (perhaps misfounded) confidence that if I just keep studying my sentences deck and reading, when I go back to that after some time it will make sense.

Right now I am reading Scott pilgrim vs the world its kinda tough but a lot of fun. I also have seen the movie a million times so that makes it a little bit easier. The hard thing about it is there is on furigana. @Aikynaro this happens not only with books but sometimes with things I hear as well.
