When I was actively adding/reviewing, I just added to the core6k deck I had. If you want to change it up now that Anki 2 has easier multi deck support then do that. I just made sure to tag everything accordingly, that was the important part so I could remember the book I got it from. I also made a separate model for my personal added cards that included page numbers in case I needed to go back and look at where I got it. I mimicked the format of my core6k deck cause it was optimal to me.

I take the picture of the sentence with my finger pointing at it with my iphone. i look it up whenever the hell i feel like which isn't often so i have like a 1000 pictures right now lol. I can add 20 cards just from watching a ajapnese talk/variety show or reading the internet... so it's not that important that i look up all these sentences asap.
I don't think you shoudl go back to the book in my opinion because there's probably better books (enjoyability wise etc) or book that are of more value to you (that you just haven't read so you don't know yet). That's the way i look at it.
