How do you know if 怒る is read いかる or おこる ?

Technically you don't; most of the time it's おこる unless it's 怒り (even then it's sometimes おこり). いかる is kind of a literary word.
Here's what goo says:
[用法]おこる・[用法]いかる――「父親は息子のうそにおこって（いかって）殴りつけた」のように、日常的な怒りが行為や表情となって外に現れる場合には、ほぼ共通して使える。◇抽象的ないかりの場合は「政界汚職にいかる」のように用い、「おこる」はふつう使わない。また、「いかる」は文章語的でもある。◇類似の語「しかる」は、相手の言動やあやまちなどを強い調子で責めること。「父親はうそをついた息子をしかった」のように用いる。

怒る i've heard おこる several times but いかる seems like an outdated reading and i've yet to listen it anywhere.

The 大辞泉 dictionary has a little note about exactly this question:
[用法] おこる・いかる――「父親は息子のうそにおこって（いかって）殴りつけた」のように、日常的な怒りが行為や表情となって外に現れる場合には、ほぼ共通して使える。◇抽象的ないかりの場合は「政界汚職にいかる」のように用い、「おこる」はふつう使わない。また、「いかる」は文章語的でもある。
My translation: おこる・いかる--as in "the father, angered by the son's lie, hit him", both can be used for when everyday anger appears in actions and expressions in about the same way. For abstract anger, such as "he was angry at political corruption", おこる is not normally used. Also, いかる can be literary....
So, both of them mean the same normally (you can read it as either), but いかる is used more in formal contexts.
