Mine is 借 (borrow) with 13 fails, but I've finally gotten it to box 5. I kept confusing it with 貸 (lend) :x
((Also I'm not sure if 13 is exceptionally high, but if it is, it's probably because I've fallen completely off the rep wagon on two separate occasions. Hehe.))

I have two that I have failed the most.
The first is 械 contraption. I keep mistaking it for other Kanji, especially 操 maneuver and 揚 hoist, because the story I came up with is too similar. I failed it 5 times, passed it 9 times, and is currently in box 5.
The second is 麦 barley. Before moving to the Heisig method, I picked a book that let me learn the first 250 kanji in native learning order. So I've learned first and second grade Kanji through brute force, so that I could at least manage myself a bit with beginner texts. Barley is simply the one that didn't stick to my head the most. I've failed it 5 times, passed it 10 times, and is currently also in box 5.
I got 17 Kanji behind that I've failed 4 times, all at least in box 4. Things are going quite smoothly for me. Nowadays, the Kanji I fail are usually recently learned Kanji.

For me it's a three-way tie between 総 (general), 岳 (point) and 握 (grip). I've failed each of them four times. I have 25 kanjis that I have failed three times as well.

This one: 粒. I've got no idea why I've failed this one 11 times...
The shared second place consists of 踊, 絡, 微 and 常 with 9 fails. All of these have finally stuck now though.
I have been reviewing RtK ever since I finished it last September.

Yeah @Stian 微 was tought for me, too. Recently, my reviews have really shrunk for RTK. It's under twenty a day now. Every once in a while I add the next few kanji from RTK3 but I'm in no rush there.

@tashippy: 微 finally stuck for me when I learnt the word 微笑む. I have forgotten way too many mnemonics. :p
For some reason, it jumped up to 30 a day straight after I moved my RevTK data into Anki (no offence, Admin; I just prefer to have everything at one place)...and today it bounced up to 50. I've got no idea what's going on.
And I think I've only got three RtK3 kanji: 狙, 誰, and 俺.
@haplochant: Go to the browse screen and enable the category "failed" and sort by it.

My most failed are 嘆 and 程, both with 8 fails. "Sigh" kept getting me because the keyword is a bit vague. It finally stuck after I re-did my story to include the famous "Bridge of Sighs" in Venice (which I've seen, so I have a nice image for it too). 程 used to get me because I kept mixing it up with another kanji, whose keyword escapes me now :p
Usually my most failed ones are the ones with the less concrete meanings, or ones I didn't have as strong a visual for.

Here's my top 5 from RTK1:
57 lapses - 徳 - benevolence; currently 2.1mo interval
48 lapses - 慕 - pining; currently 2.5mo interval
45 lapses - 誘 - entice; currently 27d interval
45 lapses - 雑 - miscellaneous; currently 3.2mo interval
42 lapses - 傑 - greatness; currently 1.3mo interval
I don't really have trouble with a lot these now, though, since I learned a lot of vocabulary to go with them by reading and studying for Kanji Kentei.
Here's my top 5 from RTK3:
32 lapses - 凌 - pull through; currently 23d interval
25 lapses - 填 - stuff up; currently 7d interval
22 lapses - 黎 - tenebrous, dawning; currently 5d interval
21 lapses - 倦 - fed up; currently 7d interval
20 lapses - 檜 - Japanese cypress [fancy fancy fancy]; currently 3d interval
I still have trouble with a lot of these, and a few of them are easily confused with other kanji that have similar meanings. With the last one, though, it's an issue of the top of it being a full roof instead of a partial one like 噌. This is why those keywords are different from the ones you probably know for them. At some point I just started making up my own keywords, and giving hints to differentiate.
It would be really neat if Fabrice could give site-wide statistics on stuff like this. It would be really interesting to know what the most difficult to grasp kanji in RTK and RTK3 are.

Such a pair helped me with 秘 and 密 which I kept confusing a really long time ago. ひ密 on one card and 秘みつ on the other card. Give it a chance.

I used to have trouble associating 密 too closely with 秘密 so I'd always forget what words like 密着 or 密度 meant.

For 密, my image was always associated with the city of Petra, carved into the cliff faces (they sorta look like mountains), so I didn't get those two confused. I imagined a city shrouded in secrecy, with the houses carved out of the mountains...

For me it is "ring" and "send back." I don't even have to look at my stats to know that!
I don't know what it is about these kanji. They drive me nuts though. I need to work on better stories for them.
I've taken a break from RTK now. I waited so long to do reviews (I think it's been over a year since I stopped at 1400 something...ooops), that I'd like to just start over fresh. At this point I'd probably fail at least 70% of the cards. My plan is to finish Japanese for Everyone, then do RTK again (should be quick until I get to the 1400s), and I'll try to read Yotsubato for fun at the same time. After RTK is done it's on to Tobira and more reading :-).
